Form # for current youth application

I am the new committee chair for our troop. I have new scouts joining this week. I am making "welcome to our troop" packets. And I am not sure which youth application to use. I inherited many, many youth applications from the previous chair and wonder which application is the most current. Is there somewhere I check the form number to see?

There's (If I recall correctly) a revision date - month/year - printed at the lower right of the front or back cover.

Maybe someone with forms in front of them can help.

FYI, per National... "Destroy all obsolete editions of Youth/Adult Applications, which are unusable."

And BTW, the current Adult Application is: 524-501A.

(Note: Be wary, Scouting.org still hosts the 28-501E, which is a previous version of the Adult Appl.)
