SLIP

Our troop has decided to require an event permission slip be
filled out for each activity. However there has been some
debate as to what information is required on this form? Here

Our permission slip is in 2 parts... The top half has all the
information as to where the scouts are going, cell phone
numbers of leaders, phone number of nearest ranger station,
when they will return, etc. That is left with the parents.

The one we use has participants name, address, phone number
and emergency contact info. Medical info. Ours also has a
place for the parents or guardian sign allowing the outing
leaders to make decisions regarding medical care in the event
of an emergency.

It is very important to stress to parents that non emergency
calls will not be tolerated. They cannot call to see if
"Johnny Misses them". I have seen parents call to check up on
Johnnny every day. (Johnny did not attend the next away trip.)

That's very true. Your scoutmaster and any new parent should
discuss in a meeting like setting about this kind of thing. We
had a scout's mother this year (dispite all the ASM's and SM's
warnings tell their kid to call home every day at summer camp
this year. Luckily the kid had a great time and actually didnt
get as home sick as I had feared. Phone calls home and from
home are your worst enemy!

I have never seen anywhere exactly what if anything that the
BSA requires to be on a permission form? I know the standard
stuff...but is there anything specific?

Bro. "Guneukitschik",

From Chapter XI of the Guide To Safe Scouting, "It is
recommended that unit leaders have a complete medical history
and permission slip for every participant attending each
Scouting activity. The medical history form and permission
slip, in most cases, will allow emergency medical treatment to
a youth member in case of injury or illness when a parent or
guardian cannot be contacted."

And, here it is, the official BSA Form 19-763...

"Consent Form: Approval by Parents or Guardians"...

By the way, the form also contains a "Medical Release" part.

We have a medical form for each scout in our Troop and we have
used the medical release section on our Troop form check it
out at the link given above.

Medical Consent

We use both a permission slip and a notarized medical consent.

We went to a year long option to cut down on paperwork. Some
will tell you this is not good enough. Some will tell you a
waiver isn't worth the paper it's printed on.

We also retain the back page of the Scout's application.

I have looked over this thread and see that there are several different formats mentioned as well as approaches to permission slips and medical releases.

So before I begin patterning one for our Troop after one of these, does anyone have additional thoughts/information/experiences that they would like to share with me? It would be especially helpful to hear from anyone with a legal perspective or someone who has had either a positive or negative experience with their forms.

At summercamp last year one of my scouts had to be taken to the hospital for a bee sting. We had all the forms from the summercamp along with permission slips from the parents but the hospital still refused treatment until they were able to contacts the parents directly (this was not an emergency). Therefore, do permission slips really matter at all?

If they were told, then their son would have been there. So, I now require a permission slip for all events, such as camping, skiing, tubing, canoeing, scout camp, etc. This way the parent signs the form, then I know they know about the event.

When parents ask me about an event I tell them that they are welcome to hear about everything during our closing (announcements). Other then that I tell them that it is the responsibility of the scout to know wheat is going on and that they should check with their son, If their son does not know it is his responsibility to call his PL.
