You meant a blue card? I thought you meant something to give the boys like they might get at a museum program. Several years ago while working at a group dental practice management company, I organized a Dentistry MB class which was run by a Dentist, a Dental Assistant, and the center manager. I put together a certificate to give them afterwards that had the MB image in one corner and the company logo in another. The manager signed each one and the boys got the certificate with their floss an new toothbrush to take home.

It's probably stashed somewhere on my hard drive.
