Troop Resource Survey Form

My troop wants to distribute a Troop Resource Survey to parents and Church members; however, the form from the official BSA site is dated from 1994 and I wanted to know if anyone has something more current that I may use and edit to be troop specific?

Our council web site has the same form but it shows as a 1997 edition.

I just tell my parents what they have to do.

Psst MT, here is a link to the Resource form Venturing Crews use, I know it has Venturing lanuage all over it, but I think with minimal editing it would work for a troop as well
